Acquired hemophilia (AHA) is related to the presence of autologous factor VIII inhibitors. AHA has an estimated incidence of about 1/1,000,000/year and a bimodal age distribution. Although rare, AHA should be suspected when a patient suddently develops a bleeding diathesis, and laboratory tests show an unexplained prolonged aPTT [1]. We report the case of a 98-year-old man admitted to our medical ward with the diagnosis of persistent haematuria, without other related symptoms. His primary care physician had started therapy with Ciprofloxacin, with no benefit.
