The introduction of intraoperative ultrasound (IOUS) in resective hepatic surgery has contributed to significantly refining diagnostic methods both in relation to screening for hepatic metastases and for the study of primary malignant tumours in which it provides a guideline for the surgeon carrying out the hepatic resection. IOUS enables direct imaging of the liver to be obtained, thus eliminating the problems related to topographical localization of lesions and allowing the exploration of lesions which were scarcely explorable at the time of preoperative ultrasound; in addition, it enables intraparenchymal lesions measuring only a few millimetres to be identified and to analyse the links between the latter and the vasculo-biliary structure, thus allowing the surgeon to modulate the width of hepatic resection.
