Background: Exercise has positive psychophysical effects on dialysis patients and effective programs should be identified. The study evaluates acute and long-term effects of an original 6-month walking program on physical capacity, health-related quality of life (HRQL) and post-dialysis fatigue (PDF). Methods: Thirty-one dialysis patients (19 males, age 65±11) were included into 2 groups: prescribed exercise (E) n=17 and control (C) n=14. Patients were evaluated on entry, after the 6-month program and 19±3 months later. Outcome measures were: 6-Minute Walking Distance (6MWD), SF-36 scale scores, self-reported PDF intensity and recovery time. E group: Two daily 10-min home walking sessions on the non-dialysis day at a speed 50% below the determined maximal treadmill speed, monthly updated at hospital . C group: No additional exercise Results: Twenty patients (E=13,C=7) completed the study. E group well tolerated exercise and, unlike the C group, increased 6MWD (308±105 to 351±118m, p=0.0007), improved HRQL, significantly for Bodily-Pain, Physical Role, Mental Health (p<0.05),decreased PDF score and recovery time (p=0.025). At the follow-up fifteen patients were re-evaluated (E=9,C=6). 6MWD values were similar to the baseline for E patients, still active, and reduced in C (P=0.026), with a decline from the baseline significantly different between groups (E=0.13±1.7,C=3.43±3.2m/month; P=0.012). HRQL, PDF and recovery time showed slight variations, almost not significant, from baseline and between groups. Conclusions: In dialysis patients, a 6-month exercise program prescribed at hospital-performed at home positively affected HRQL, PDF symptoms and physical capacity, whose decline was slower over a two-years period compared to controls .
