The authors describe a case of diffuse bilateral necrotizing PCP occurred in a HIV negative patient who had at computed tomography of the lungs bilateral interstitial infiltrates, peribronchial thickening and subpleural and intraparenchimal cysts only present within the lung infiltrates. Analysis of BAL fluid revealed the presence of P. carinii. Results of a gallium-67 scan were compatible with a diagnosis of PCP. The findings support the hypothesis that a more virulent strain of P. carinii may be responsible for this case. The preexisting pulmonary pathology with impairment of lung defense may also explain the occurrence of PCP.
