Purpose To present a new surgical technique for scleral fixation of the Smaller-Incision New- Generation Implantable Miniature Telescope (SING IMT) using 3 Gore-Tex sutures. Design Case report. Subject An 85-year-old female with bilateral late-stage atrophic age-related macular degeneration underwent intraocular lens removal and SING IMT implantation in her left eye. The first postoperative day, dislocation of the SING IMT device was noted. Scleral fixation of the SING IMT was performed. Methods The SING IMT was explanted through a superior 8 mm limbal incision. Both ends of 3 double armed 8-0 Gore-Tex sutures were passed through each of the 3 haptics of the device and through 6 sclerotomies created 2.5 mm posterior to the limbus. The SING IMT was re-inserted through the main incision and the 3 Gore-Tex were tied to secure the devide to the sclera. Posterior vitrectomy was performed by visualizing the posterior segment through the spaces between the haptics. Results Postoperatively, the device remained well centered through 6 months of follow-up. Conclusions and Importance Scleral fixation of the SING IMT with Gore-Tex sutures represents a safe and reliable method for securing the device in cases without adequate capsular support. This technique opens up the possibility of implanting the device in pseudophakic eyes.
