The existence of a high level of eterogeneity in morphology and localization of periodontal intrabony lesions implies a different level of surgical accessibility for each defect, influencing clinical outcomes when a surgical reconstructive procedure is undertaken. The present paper illustrates step by step a new surgical technique for periodontal reconstructive surgery based on the single flap approach, that is the elevation of a single flap (exclusively buccal or exclusively lingual/palatal), keeping intact the tissues on the opposed side.
