We report a case of a patient suffering from acute low back pain radiating into the left leg, due to a pyomyositis of the iliopsoas and piriformis muscles, extended to the gluteal region through the greater sciatic notch. Although rare in temperate climate, pyomyositis should be borne in mind in patients with fever and severe, incapacitating pelvic pain or radiating ‘‘sciatica’’, or in obscure cases of sepsis, as early recognition and treatment are essential to prevent a fatal outcome.
