We describe the case of a 69 year old woman presenting clinical features mimicking a severe asthmatic exacerbation at the emergency room of our hospital. Chest imaging showed the presence of a mass inside the tracheal lumen. The patient initially underwent endoscopic resection by rigid bronchoscopy followed by conservative tracheal surgery. The final histopathological diagnosis was tracheal adenoid cystic carcinoma. After more than 4 years of follow-up, no signs of local relapse and/or metastases were present
