The tipical picture of hanging is the furrow wich configuration depend on two factors: the material used for constriction of the neck and the time of the body’s suspension. So, when ropes, electrical cords or belts, are used and the body has been suspended for a long time, we can observe a dark brown, deep and well demarcated furrow often with a mirror image of the rope on the skin. Instead soft materials like a t-shirts, clothes or silk pyjamas and a short time of suspension might produce a pale soft and poorly defined furrow with the upper margin red, often discontinuous and not even visible at the gross examination. In these cases it’s a very difficult problem to distinguish a soft furrow from the skin’s post-mortem modifications and sometimes neither the classic microscopical examination (E/E) might be a diriment answer at the question because the hematic infiltration on the dermal or subdermal tissues is very poor or absent. In this study we investigated 12 autopsy cases of death due to hanging with a soft materials and we observed in skin specimens the immunohistochemical expression of a panel of cytokines and inflammatory cells for a furrow vitality estimation. The microscopical observations showed a strong positivity of CD 15, tryptase and IL15. These reactions and above all the presence of IL 15, a proinflammatory cytochine, denote the furrow’s vitality.
