TNF-a production has been investigated in bronchoalveolar lavage fluids (BAL) obtained from patients with AIDS and Pneumocystis carinii pneumonia (Group A),; AIDs and IIDS related complex (ARC) without PCP but with evidence of P. carinii in their specimens (Group B); and seropositive patients with asymptomatic HIV infection without demonstration of Pneumocystis (Group C). TNF-a were determined in supernatants from cultured alveolar macrophages (AM) by using a sandwich ELISA. Increased TNF-a values were detected in supernatants from Groups A and B whose concentrations were significantly higher than those observed in symptomatic HIV+ patients or in those with ARC. Furthermore, the increased TNF-a production from Group A was associated with lower P. carinii counts than those found in patients from Group B. These results seem to indicate that BAL TNF-a production from patients with AIDS and PCP is not different from AIDS or ArC colonized by P. carinii and that this cytokine does not discriminate between P. carinii infection of colonization.
